Description
Oak Street Health Home Visit NPs deliver high-touch, high-quality primary care to our homebound patients. This role is supported by a scribe / medical assistant, nurse, social worker, and other resources. NPs make 6-7 visits daily in a defined territory and collaborate continuously with other team members for care coordination and all other needs.

The typical pay range for this role is :
95,738.00 - $206,206.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
