Nurse Care Manager -LPN

As a Nurse Care Manager LPN for Swope Health Services, you will be a part of a team you will be part of a team of caregivers to provide care for the people who need it most. Our Nurse Care Manager manages a wide array of physical health needs, and behavioral health needs. If you have a passion for working with a team of dedicated caregivers to make care visible every day, this role is for you.

Successful Candidate will have :

• Current, unrestricted license as LPN in the State of Missouri and Kansas, BSN preferred.
• License to start, willing to hire LPN without Kansas license if eligible to obtain Kansas license within the first 90 days of employment.
• 2-5 years of experience in an outpatient clinical setting or previous experience in care coordination role

pediatric experience preferred.
