Requirements Active TX State Medical License, DEA (with TX address), Board Certified / Board Eligible in ABEM (or osteopathic counterparts), Must be residency trained in Speciality Certified in , ACLS, ATLS, & PALS, If ABEM, need current ACLS and 1X proof of ATLS , CME past 2 years, case logs past 2 years, copies of all education & training diplomas, SS Card, Driver's license, color photo emailed to MSO, TB / FLU / Hep B / MMR / VARICELLA Immunization records. Annually must have 8 hrs of Stroke CME and 8 hrs of Trauma CME. Must take in person EMR Training prior to first shift- PCC Coordinates- does not have to be privileged prior to training Cross credential all providers at THOP Montwood and Horizon City.
