American Hospice and Home Health Services is on the lookout for a dedicated Full-Time or Part-Time Speech Therapist / Speech-Language Pathologist (SLP) to join our compassionate team of professionals.
We serve multiple counties including Alameda, Contra Costa, Solano, San Joaquin, and Stanislaus.
We offer competitive compensation with a choice of per-visit pay or hourly / salary options based on your experience.
Per Visit Rates : $140 - $220 per skilled speech therapy visit
Hourly Option : $50 - $65 (during office hours)
The Speech Therapist will play a crucial role in assessing, planning, implementing, and monitoring tailored speech, language, swallowing, and cognitive therapy services for our home health patients. Close collaboration with nursing, physicians, caregivers, and therapy staff is essential to provide high-quality, patient-centered care.

Key Responsibilities :
Evaluation & Assessment :
- Conduct thorough assessments of speech, language, voice, swallowing, and cognition in patients’ homes.
- Review medical records and collaborate with nursing to incorporate patient history in evaluations.
- Assess the home environment to enhance therapy outcomes (safety, availability of assistive devices, caregiver support).
- Identify needs, preferences, and goals of patients, families, and caregivers.
Treatment Planning & Implementation :
Create individualized, measurable therapy plans aligned with patient goals, incorporating family input and physician orders.Deliver direct skilled therapy (speech, language, swallowing, cognitive) to patients.Adjust treatment plans based on ongoing patient progress and response.Educate and train caregivers on home exercises and strategies to support therapy goals.Work collaboratively with an interdisciplinary team using Google Workspace applications and the HomeHealthMD EMR system to ensure cohesive care.Documentation & Compliance :
Complete all required documentation (evaluations, treatment notes, progress updates) according to agency policies and regulatory timelines (within 48 hours).Accurately document therapy contacts, missed sessions, cancellations, and outcomes.Ensure all therapy elements are adequately recorded for billing, compliance, and justification purposes.Communicate any changes in patient status or care plans to physicians and the care team.Communication & Collaboration :
Engage in care team meetings, case discussions, and interdisciplinary conferences.Report therapeutic findings and concerns clearly and promptly to physicians, nursing, and leadership.Provide respectful and timely communication with patients, families, caregivers, and staff.Additional Duties :
May be required to provide on-call therapy as agreed within agency policies.Assist in scheduling, travel coordination, and therapy resource management.Engage in ongoing professional development to maintain licensure and competency.Qualifications :
Valid Speech-Language Pathologist license in California.A minimum of 1 year experience in home health, outpatient, hospital, or similar settings is preferred.ASHA certification (CCC-SLP) or relevant credential.Current CPR / BLS certification.Reliable vehicle, valid driver's license, and appropriate insurance are required.Excellent communication, organizational, and documentation skills.Work Location & Travel :
