Job Title : Case Manager

Experience : Minimum of 2 years in acute care coordination experience

License : Current nursing license required

Certifications : BLS

Must-Have :

• Proficiency in Midas, InterCall, Cerner, Curaspan, and Microsoft Office programs
• Experience with discharge planning and utilization review
• Prior acute care experience

Description :

The position requires ability to work every third weekend.

Candidates should be adaptable, flexible, and capable of quickly integrating into the team.

Excellent attendance is expected.

Taking breaks and lunches is necessary.

This role involves a patient assignment ratio of 1 : 20.

Candidates must be willing to cover all adult areas of the facility.

If there is no experience in women's health, covering that area will not be required.

Business casual conservative dress is required; no open-toed shoes, minimal fragrance, and a professional appearance is expected.

Care coordination will involve collaboration with other healthcare professionals.

Familiarity with hospital charting systems and protocols is essential for success in this role.
