Family Practice / Primary Care Nurse Practitioner

Are you a seasoned Nurse Practitioner passionate about relationship-based care for older adults? A leading value-based primary care group is launching services at a new senior living facility in the Greater Boston area and is seeking an experienced NP to lead clinical operations on-site.

Position Highlights :

• Serve as the primary care provider for a new senior living community
• Deliver thoughtful, unrushed care to a stable panel of older adult patients
• See 8 10 patients / day in a supportive, low-volume setting, average just over 1 visit per hour
• Benefit from virtual care team support (social workers, nurse care managers, community health workers)
