Schedule : Evenings (11pm-7am), Every other weekend

SUMMARY

Join our dedicated team as a Licensed Practical Nurse (LPN) where you'll play a vital role in providing compassionate and professional patient care to long-term and post-acute care residents. You will lead and support Certified Nursing Assistants (CNAs), ensuring high standards of care and accurate documentation are consistently upheld, contributing significantly to the wellbeing of our patients and the success of your team.

Key Responsibilities :

• Administer medications promptly and accurately, maintaining thorough documentation and compliance.
• Perform treatments and document patient progress according to physician orders and facility protocols.
• Utilize computerized medical record systems for precise and complete documentation of resident care.
• Guide CNAs in delivering individualized personal care in alignment with resident care plans, ensuring their documentation is accurate.
• Communicate relevant information to appropriate staff through verbal and written reports, adhering to departmental protocols.
• Contact physicians as necessary, transcribing orders and implementing them promptly.
• Monitor residents’ health and report significant changes to the charge nurse or physician immediately.
• Complete pre-reviews of medication administration records in line with facility protocols.
• Timely completion of required MDS documentation and related observations.
• Support the CNA team by participating in direct care activities for residents as needed.

Medium Work - Ability to exert varying amounts of force frequently and occasionally, including moving objects. Frequent walking, standing, and squatting may be required.

The listed base pay range reflects the potential base pay for a successful full-time applicant. Adjustments may be made based on experience, qualifications, specialty, and internal equity.
