Collaborate with a team of 15 full-time and 17 part-time social workers. Handle a caseload of 15-20 patients / consults per day. Consult with respiratory therapy and pharmacy for discharge planning as needed. Participate in department-specific orientation for 2 weeks. Report directly to the Social Work Manager, with oversight provided by the Chief of Care Coordination. No weekend requirements; holidays are rotated as needed. Maintain a professional appearance, wearing any color scrubs or business professional attire. Join a growing team aiming to expand from 15 to 21 case managers.

License : RN License required

Must-Have :
- At least 2 years of acute care management experience in discharge planning.
- 6 months experience in a level 1 or large academic medical center hospital.
- Proficiency in EPIC charting.
