Stonegate’s Central Intake Department has an excellent opportunity for an experienced high-volume Admissions Coordinator .  As the Central Intake Admissions Coordinator

in Lewisville, TX , you will support and work out of the DFW Central Intake Center.

The Central Intake Admissions Coordinator will be responsible for the intake and processing of referrals to ensure timely turnaround as well as communication with referral sources and staff of skilled nursing facilities to ensure smooth transition from hospital to Stonegate affiliated properties.

Job Requirements

Experience reviewing a high volume of hospital referrals for SNF and / or LTC placement.

Experience working in various hospital referral portals.

Must have excellent interpersonal and telephone skills and the ability to relate professionally and positively to referral sources, physicians, patients, patient families, and other team members and consultants at all levels.

Must be detailed oriented and possess the ability to multitask.

Proficient computer skills

RN or LVN in good standing in the State of Texas- is a plus, but not required.
