Behavioral Health Care Coordinator

A company is looking for a Behavioral Health Care Coordinator to support their Behavioral Health Clinical Support team through telephonic outreach and care coordination.

Key Responsibilities

Conduct outbound phone outreach to engage members in behavioral health services

Perform behavioral health screenings and assist with care transitions

Coordinate referrals and link members to community resources and appropriate services

Required Qualifications

Experience in a high-volume call center, administrative office, or healthcare support environment

High School Diploma with substantial experience in behavioral health coordination; or Associate degree in a health-related field; or Bachelor's degree in psychology, social work, or a related discipline (preferred)

Customer service or call center background preferred

Experience coordinating behavioral health or substance use disorder services preferred

Bilingual or multilingual abilities are a plus
