Be a part of our new, exciting “small house concept” skilled nursing community located in Homer Glen, IL We are currently seeking a Social Services / Admissions Coordinator, Full-Time position. Responsibilities :
- Work directly with the Director or Social Services to manage a caseload of 34 short-term and 20 long-term residents.
- Complete initial assessments to plan for each resident’s needs while they receive skilled nursing and therapy services, as well as upon discharge.
- Plan for safe discharge placement and coordinate continued services for each resident.
- Complete MDS assessments, care plans, and all necessary documentation.
- Promptly handle resident concerns and follow up with the resident / family.
- Participate in interdisciplinary team and care plan meetings.
- Assist with maintaining census.
- Respond to inquiries.
- Enter admitting patient information into Point Click Care.
- Obtain insurance authorization for admitting patients.
- Track incoming referrals through a CRM tracking tool.
- SNF experience required.
Requirements
Works closely with Administrator, Marketing Manager, and / or Clinical Assessment Coordinator.Must be able to read , write, speak and understand the English language.Must possess the ability to make independent decisions when circumstances warrant such action.Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies / personnel, and the general public.Must possess the ability to work harmoniously with other personnel.Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care and maintaining a sound operation.Must be thoroughly familiar with the laws, regulations and guidelines governing the release of information.Must be able to maintain good personnel relations and employee morals.EOE
