We are looking for a committed advanced practice clinician (NP or PA) to guide the care and long-term management of individuals living with heart failure through virtual oversight and medication adjustment. This role centers on refining therapy plans, interpreting remote physiologic data, and partnering closely with cardiology colleagues and a multidisciplinary support team to strengthen patient outcomes and limit avoidable admissions. The position combines the convenience of working from home with meaningful patient interaction in a mission-oriented organization dedicated to elevating heart failure care.
Role Summary
Therapeutic Management :
Direct the rollout and fine-tuning of evidence-based heart failure treatments. Adjust medications through accelerated titration schedules aligned with established pathways, using clinical reasoning to adapt therapies based on symptoms, laboratory values (kidney function, potassium, etc.), and digital health information.
Virtual Monitoring & Patient Support :
Leverage HekaHeart's digital tools to follow patient trajectories, including vital signs, hemodynamic data (e.g., CardioMEMS), and lab results. Address urgent notifications promptly and connect with patients to deliver timely, individualized care touches.
Team Collaboration & Leadership :
Work hand-in-hand with HekaHeart nursing and medical staff to ensure a smooth, consistent virtual care experience. Provide direction and clinical insight to maintain a unified and high-quality approach across the care team.
Protocol Improvement :
Participate in shaping and revising clinical workflows for the remote clinic, ensuring processes continue to evolve to enhance efficiency and patient well-being.
