Job Overview
The Community Health Worker (CHW) Supervisor reports directly to the CHW Manager, and is responsible for overseeing the CHW team and their functions. The CHW program is part of AltaMed’s innovative, evidence-informed care management program that improves health outcomes for vulnerable and high-risk patients. In this program, CHW Coordinators and CHW’s provide structured advocacy, social support, and navigation to high-risk patients in order to help them reach optimal health. CHWs are trained laypeople who share life circumstances (race, socioeconomic status, language, etc.) with their patients, and are natural helpers within the community. The CHW Supervisor reports program / staff needs to Manager, provides supervision, coaching, and direct support to the CHW team, and monitors the progress of the program / case management. The CHW Supervisor provides leadership to the team through training, mentoring / professional development, case discussion / reviews, and more.
The CHW Supervisor demonstrates an advanced level of communication and interdisciplinary collaboration, professionalism, and systems-based thinking with an emphasis on the importance of patient-centered care, education, and organizational involvement to meet the team’s identified goals. The CHW Supervisor will effectively communicate feedback and provide ongoing coaching and mentoring to staff while supporting a learning environment to advance team skills and promote professional development while working closely with Lead CHW.
Minimum Requirements
1. High School Diploma or GED is required. AA / AS preferred.
2. Minimum of 3 years of experience directly working in the health care environment or a community outreach setting required.
3. Bilingual (Spanish / English) or another secondary language required due to population being served.
