Responsibilities

Administer nursing care to ill, injured, or disabled patients
Deliver care according to patient treatment plans
Monitor and report changes in patient symptoms or behavior
Communicate with physicians or specialists regarding patient care
Educate patients on health maintenance and disease prevention
Facilitate referrals to other healthcare professionals and facilities
Maintain accurate medical records
Provide emotional support to patients and families

Qualifications

Active Georgia RN or LPN license

Minimum one year of home health care experience

BLS Provider CPR certification

Experience with chronic disease management (preferred)

Self-motivated, dedicated, and proactive

Experience with feeding tubes, tracheotomies, or ventilators (a plus)

Strong problem-solving and critical thinking skills

Ability to work in a fast-paced environment
