Registered Nurse - FT East Valley

REPORTS TO : Director of Nursing

SUMMARY : Provides skilled professional nursing services to a case load of patients in their place of residence in accordance with the patient's plan of care.

DUTIES AND RESPONSIBILITIES :

• Assesses patients prior to provision of care.
• Provides ongoing assessment of the patient and patient's equipment needs.
• Initiates timely discharges.
• Educates and instructs the patient, patient's family, or staff as required; counsels patient and family in meeting patient's needs and goals.
• Prepares timely, legible, and complete documentation of all care, interventions, and care coordination as provided by law, regulation, and established policy; returns documentation to office / department according to established policy.
• Demonstrates an awareness of and sensitivity to patient / family rights.
• Applies safety principles as identified by established policy.
• Participates in the orientation and training of new staff.
• Participates in care coordination meetings.
• Participates in continuing education to continually improve skills and abilities and stay abreast of current technologies / practices.
• Ensures compliance with legal issue including but not limited to patient confidentiality and risk management; ensures compliance with JCAHO, federal, state, and local regulations.
• Exhibits a high degree of courtesy, tact, and poise when interacting with patients, families, and other healthcare professionals.
• Adjusts to fluctuating peaks in patient flow, acuity, and other operational demands while maintaining quality.
• Participate in the Performance Improvement program as requested by the Performance Improvement Coordinator.
• Performs other related duties as assigned by management.
