Conducting case management activities as well as discharge planning and maintains appropriate documentation. Providing appropriate therapeutic and educational interventions, services and is knowledgeable about community resources. Performing other duties as assigned. This role requires a Master's prepared in the field of Clinical Social Work, Mental Health Counseling, or Marriage and Family Therapy, or other related field. BLS (Basic Life Support) Driver's License Driver's License LMFT (Marriage and Family), LMHC (Mental Health) or LCSW (Social Work) - Preferred Master's in Social Work, Psychology or Related Field 2 years clinical related experience or 9 month internship. - Required 1 year of relevant experience with Master's - Preferred
