Job Description
General :
This position, under the supervision of the Director will assist the program staff with case management administrative duties and client record keeping.
Job Requirements :
- Maintaining client records in accordance with established policies and procedures
- Coordinating, monitoring & documenting relevant issues for clients
- Providing technical support for program staff, including client intake
- Serving as a liaison between the professional staff, community agencies and clients
- Participating in all mandated staff development sessions
- Assisting with community resource meetings
- Facilitate transportation for clients
- Monitor utilization of program resources
- Contacting institutions to confirm resident departure times
- Generating program reports to track program services
- Tracking client participation
- Performing other duties as assigned by Program Personnel
Qualifications / Skill Requirements :
One year of related experience in an office environment.Abilities :
Understand and comply with company and contract agency rules, regulations, policies and procedures;Communicate effectively and all levels within and outside the organization;Express ideas clearly and completely, written and oral; Manage time to meet the responsibilities and duties of this position;Give attention to detail and follow-through;Establish and maintain appropriate working relationships with co-workers and contracting agency representatives;Execute all duties in accordance with this position description.Education requirements :
High School or Equivalent
