Wellness Nurse Full Time 2500 Sign on Bonus!

Salary : $35.00 - $37.50 per hour  $2500 Sign on Bonus!

Schedule : 2 Full Time Schedule Options : 1) Sunday to Tuesday Sunday to Wednesday (3 on 4 off 4 on 3 off) 2) Thursday to Saturday Wednesday to Saturday (3 on 4 off 4 on 3 off).

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support provides assistance to community nurses caregivers and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.

• Assist in coordination of the resident admission process including screening assessment and care needs
• Ensure care is delivered as indicated in resident care plans
• Medication and treatment administration and ensure medications are administered per physicians orders
• Assistance with coordinating care and services with other outside providers including resident physicians
• Document and report care provided and resident observations including unusual or significant changes
• Maintain a working knowledge of state regulations policies and procedures
