What Youll Do

As part of Strives interdisciplinary clinical care team the N urse Practitioner (NP) will utilize theiradvanced education skills and judgment to deliver advanced nu rsing carefor CKD and ES K D patients. The NP will be responsible for managing patient care by working with the Strive interdisciplinary team of social workers care coordinators pharmacists dietitians and physicians to provide best in class patient Strive NP will be anadvocate navigator trusted clinician and safety net for the patientwhile delivering exceptional clinical care building empathy and trust with patients and delivering a world class patient experience . This position reports to the Lead Nurse Practitioner .
