Senior Social Worker - Interim
New York
Position Description
The Interim Senior Social Worker provides both clinical services and program leadership within outpatient clinics and limited inpatient areas. This role blends direct practice with oversight of a small team (3 social workers plus one isolated role) and supports the stabilization, evaluation, and improvement of a rapidly growing program.
Program Leadership & Supervision
- Provide clinical supervision, mentoring, and support to social work staff.
- Lead program development, workflow improvements, and quality initiatives.
- Participate in interdisciplinary meetings and collaborate with medical practice leaders.
- Support onboarding, skill development, and documentation standards.
- Maintain strong relationships with community agencies and promote effective referral pathways.
Clinical Practice
Conduct psychosocial assessments and provide counseling, crisis intervention, and care planning.Collaborate with physicians, nurses, and care managers to coordinate care and ensure smooth transitions.Address social determinants of health (housing, transportation, financial assistance, etc.).Arrange post-hospital or specialty services (rehab, home care, hospice, long-term care).Complete timely documentation and ensure compliance with HIPAA and regulatory standards.Consultation & Teaching
Provide guidance on complex cases to social workers, care managers, and clinical teams.Assist in policy development and best-practice standards for social work services.Participate in audits, performance improvement, and risk management activities.Qualifications
Education & Licensure
Master's degree in Social Work (MSW) — requiredLicensed Clinical Social Worker (LCSW) — requiredExperience
Minimum 3 years of hospital or medical social work experienceExperience with crisis intervention, care coordination, and discharge planning.Supervisory or leadership experience required.Experience in rapidly evolving or developing programs is a plus.Skills
