Schedule : Weekends and half days during the week.

Position Summary :

We are seeking a dedicated and compassionate Physical Therapist to join our dynamic rehab team on a per diem basis. The ideal candidate will provide individualized therapy services to help residents achieve their highest level of functional independence.

Responsibilities :

• Evaluate, plan, and implement physical therapy treatment programs for residents.
• Collaborate with interdisciplinary team members to ensure comprehensive care.
• Educate residents and families on therapy goals and progress.
• Maintain accurate documentation in compliance with regulatory standards.
