We want to meet our patients where they are at and we know many of our patients want to stay in the safety of their own home. We're looking for NPs to complete important annual assessments for our patients via Telemedicine (in particular, via best-in-class video technology). This role will be fully remote and the work hours are 10am to 7pm CST. We partner with a network of elite specialists and hospitals for specialty and acute care. This role is vital for OSH to deliver best-in-class primary care, care coordination, and population health. Responsibilities include :

• Provision of exceptional primary care.
• Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.
• 100% of visits via Video
• Care coordination with other providers, specialists, testing facilities, agencies (both within OSH and third party providers)
• Population health leadership, in coordination with the Care Team
• Patient education.
• Other duties as assigned
