We are currently seeking a motivated and experienced  Hospital Liaison  to join our team. The  Hospital Liaison  plays a critical role in facilitating communication and collaboration between our hospital and external healthcare providers, community organizations, and patients. This position offers an exciting opportunity to contribute to our mission of providing high-quality healthcare services to our community.

Hospital Liaison Responsibilities :

• Serve as the primary point of contact for external healthcare providers referring patients to our nursing homes.
• Build and maintain relationships with physicians, case managers, social workers, and other healthcare professionals
• Coordinate patient transfers and admissions to our nursing homes, ensuring a smooth transition of care
• Educate external stakeholders about our services, specialties, and admission criteria
• Conduct outreach activities to promote our programs and services to the community
• Gather and analyze data on referral patterns, patient demographics, and market trends
• Collaborate with internal departments to address patient needs and concerns
