Hospice RN Case Manager
Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM
Job Type : Full-Time | Field-Based Territory : Macomb County

What You'll Be Doing
Deliver high-quality nursing care to hospice patients in their homes
Perform comprehensive assessments and create individualized care plans
Monitor patient conditions and respond proactively to changes in status
Collaborate with physicians and interdisciplinary team members to coordinate care
Educate patients and families on disease progression, medication, and comfort care
Support families emotionally and practically through end-of-life care
Ensure compliance with physician orders and regulatory standards
What You Bring
Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
Current State License as a Registered Nurse RN
1 year of experience in a clinical care setting
Experience in a hospice or home health environment is preferred
50% travel required
Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
Valid driver’s license and insurance and reliable transportation to perform job tasks
