Job Description

Administer skilled nursing care for clients of all ages in their residence.

Assess home health and / or hospice patients to identify their physical, psychosocial, and environmental needs.

Implement, document, and evaluate the Plan of Care to ensure quality and continuity of care.

Provide patient education on care, diet, medications, and home safety.

Supervise the Home Health Aide and ensure continuity of service quality.

Monitor assigned cases for compliance with requirements of third party payers.

Perform on-call responsibilities and provide effective communication with patients and team members.

Critical Information

Shift : M-F until shift is complete; on-call required if requested.

Travel Area : Dekalb County.

Education / Licenses Needed

Current state license as a Registered Nurse.

Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.

CHPN certification preferred.

Proof of current CPR certification.
