This  contract role as a Community Health Worker in Chicago, IL is responsible forsupporting the IL Medicaid Care Coordination team by outreaching to IL Medicaid members to complete their initial health risk screening and completing door knocks when necessary to contact hard to reach members.

Pay Rate : $26 - $29 per hour

Job Requirements :

• Bachelor Degree OR 4 years’ health care, social services or community advocacy experience,
• Knowledge of medical terminology
• Experience coordinating member related needs, providing assistance to members, and analyzing member needs
• PC proficiency including Microsoft Office applications
• Customer service skills
