Do you have room in your home and heart to support an adult with Intellectual and / or Developmental Disabilities? Are you able to support an individual to be successful at home, work, and community? If so, you may find fulfillment in the role of a Family Model Provider.

Summary : Family Model Provider : We are looking for caring individuals who share our dedication to providing a supportive environment for persons with intellectual and / or Developmental Disabilities (I / DD) in their home. We ask that you bring a desire to serve others and share your home, and RHA will provide the training and support to ensure your success.

As a Family Model Provider, you will be asked to provide support for individuals with disabilities in a home setting. You will be asked to provide the supports identified below based on the needs of the individual in your care.

Pay : $50k+ / yr

Position Requirements :

• HS Diploma / GED preferred
• 18 years old or older
• Documentation of home ownership : current mortgage statement or renter’s lease
• Proof of homeowner’s or renter’s insurance or personal property insurance
• Valid Driver’s License; Reliable transportation with valid vehicle insurance
• Ability to pass criminal background and Registry checks as well as a Drug Screen
• TB Screenings for all members of household
• Willing to be a self-employed worker
