Job Summary

We are seeking a compassionate and reliable Home Health Hospice Aide to provide end-of-life care and emotional support to patients and their families in a home setting. This role involves assisting with daily living activities, ensuring comfort, and maintaining dignity for patients during their final stages of life.

Key Responsibilities

• Provide personal care including bathing, dressing, grooming, and toileting
• Assist with mobility and positioning to prevent discomfort or injury
• Monitor and report changes in patient condition to the hospice care team
• Offer emotional support and companionship to patients and their families
• Maintain a clean, safe, and peaceful environment for the patient
• Support the hospice team in implementing care plans
• Document care activities and observations accurately
• Respect patient confidentiality and uphold ethical standards of care
