Job Description

As a Care Manager at Families First, you play a vital role in guiding and supporting our young patients through their mental health journey. Your responsibilities include welcoming and orienting patients, conducting comprehensive assessments, offering family support, collaborating on individualized care plans, ensuring effective care coordination, and developing crisis management and safety plans.

Location : Bluffdale

Type : Part-time

Qualifications

Bachelor's degree in Social Work, Psychology, or related field.
Excellent interpersonal and listening abilities.
Excellent verbal and written communication skills.
Problem solving, decision making, and critical thinking abilities.
Ability to stay calm during high-pressure situations.
Ability to work within an interdisciplinary team.
Ability to work with young people and their parents / guardians.
Empathetic, patient, and understanding.
Must be able to maintain client's confidentiality and adhere to ethical practices.
Must be able to relate with people from diverse lifestyles and cultures.

Responsibilities

As a Mental Health Care Manager at Families First Pediatrics, you will :

Welcome and create a smooth transition for patients into our care programs.

Conduct comprehensive interviews to assess patients' mental and emotional needs.

Offer guidance, support, and information to foster a collaborative approach to patient care.

Create individualized mental health plans with therapeutic goals and strategies.

Ensure effective care coordination and comprehensive support for patients.

Develop crisis management and safety plans for patient safety during challenging situations.
