Job Posting : Occupational Therapist -Therapeutic Consultation (VA DD Waiver)

Overview Community Health and Wellness (CHAW) seeks an Occupational Therapist (OT) to provide Therapeutic Consultation under Virginia's Developmental Disabilities Waivers (FIS / CL). OT is a consultative service : assess needs, create a written OT support plan, train and coach families / caregivers / DSPs, and monitor outcomes. This role does not provide ongoing direct therapy.

Responsibilities

  • Conduct OT assessments and observations.
  • Develop written OT support plans with strategies / adaptations.
  • Demonstrate interventions and train families, caregivers, DSPs.
  • Participate in annual ISP meetings and team reviews.
  • Complete Medicaid-compliant documentation (notes, reviews, training logs, summaries).

Qualifications

  • Occupational Therapist licensed in Virginia
  • Experience with individuals with ID / DD.

    • Schedule & Modality Unit= 1 hour; in-person or HIPAA-compliant teleconsultation. Frequency determined by assessed need and ISP authorization.
