Position Overview :

We are seeking an experienced and compassionate Hospice Registered Nurse (RN) Case Manager to join the interdisciplinary team and provide comprehensive care coordination and support to hospice patients and their families. The RN Case Manager will be responsible for assessing patient needs, developing individualized care plans, coordinating care services, and providing emotional support throughout the end-of-life journey.

Key Responsibilities

  • Clinical Assessment & Care Planning

Conduct physical and psychosocial assessments; develop and adjust individualized care plans in collaboration with patients, families, and the care team.

  • Symptom & Pain Management
  • Manage pain and symptoms using evidence-based practices; administer medications safely; educate patients and families on symptom control.

  • Emotional Support
  • Provide counseling and emotional support to patients and families; assist with coping strategies and end-of-life planning.

  • Care Coordination
  • Coordinate interdisciplinary care and services; ensure smooth communication and care transitions; participate in on-call coverage.

  • Documentation & Compliance
  • Maintain accurate EHR documentation; ensure regulatory compliance; contribute to quality improvement initiatives.
