Care Manager, Home Care

Elder Life Management now part of the BAYADA family is seeking a compassionate Care Manager to support our elder clients in Tinton Falls NJ and surrounding areas.

This role requires the ability to assess plan and coordinate care for older adults both at home and in facility settings while advocating for their independence and well-being.

As a Care Manager you will :

• Conduct comprehensive assessments and develop individualized care plans
• Coordinate home care services and other supports
• Attend medical appointments as a client advocate and provide ongoing support
• Act as a liaison for families at a distance
• Arrange monitor and adjust in-home services as needed
• Provide crisis intervention and problem-solving support
• Engage and coordinate community resources to enhance clients quality of life
• Promote meaningful social spiritual and cultural activities
• Maintain accurate records and documentation of client care
