What You'll Do

Evaluate home health patients for functional and ADL needs

Build and adjust individualized treatment plans

Deliver therapy visits in patient homes (Chesapeake area)

Coordinate with PT, RN, and intake teams for care continuity

Chart and track visits in WellSky (Kinnser) EMR

Ensure compliance under PPS / Medicare guidelines

Support strong patient outcomes through efficient visit use

Must-Haves

Active OT license in Virginia

1+ year of home health or adult rehab experience

Familiarity with WellSky / Kinnser EMR preferred

Reliable vehicle and valid driver's license

Current CPR certification
