What You Will Do
- Provide clinical oversight and mentorship to field clinicians, offering expert guidance, hands-on support, and real-time coaching to strengthen clinical practice and improve patient outcomes.
- Conduct in-home recertifications , assessing each child’s progress, reviewing plan-of-care needs, and ensuring continued appropriateness of services in alignment with regulatory standards.
- Perform home health aide follow-ups to evaluate quality of care, confirm task competency, reinforce care expectations, and ensure adherence to individualized care plans.
- Lead orientation supportive duties , including onboarding new clinicians, reinforcing agency policies, and helping new team members feel confident and prepared to deliver safe and effective care.
- Provide ongoing mentorship , fostering professional growth by offering constructive feedback, clinical education, and a supportive environment that encourages continual learning.
- Oversee quality assurance initiatives , reviewing documentation, identifying areas for improvement, monitoring compliance, and ensuring consistent delivery of exceptional pediatric home health services.
- Follow up on physician orders , confirming timely receipt, clarifying clinical details as needed, coordinating updates to the care plan, and maintaining clear communication between the care team and providers.
- Conduct supervisory visits to support high-quality, developmentally appropriate nursing and therapeutic care.
- Review, approve, and guide care plans to ensure accuracy, regulatory compliance, and application of best practices.
- Collaborate with interdisciplinary team members to promote continuity of care and child- and family-centered decision-making.
