Physical Therapist, PT - Full-time / Per-Diem

We are seeking an experienced Physical Therapist to join our team. The successful candidate will be responsible for providing high-quality patient care that will help change patient's lives for the better at the comfort of their homes.

The Physical Therapist evaluates and treats residents, under orders from a physician, through the use of a variety of therapeutic procedures designed to restore loss of muscle strength, coordination and body alignment, to relieve pain and to improve movement and function. The Physical Therapist participates as a member of the interdisciplinary team providing support and information within the area of Physical Therapy.

Responsibilities

• Performs physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation for clients who are receiving services under a medically approved Plan of Care.

Completes initial assessments within forty-eight (48) hours of referral unless other arrangements are made.]

Assesses knowledge level of client and identifies learning needs.

Serves on the Clinical Record Review Committee.

Demonstrates knowledge of safety / infection control practices by compliance with agency policies and procedures.

Performs in-home supervision of home health aides and physical therapy assistants as directed and according to Agency policy.

Provides services in accordance with applicable state, federal, CHAP, and JCAHO standards.

Qualifications

Association and the American Physical Therapy Association.
