Occupational Therapist ( OT)

Shifts Available : Full Time Part Time & PRN

Service Area : Crosby HighlandsAtascocita Humble Kingwood

Responsibilities :

• Facilitate rehabilitation of patients with emotional physical or mental disabilities.
• Develop and implement occupational therapy care plans.
• Coordinate care with home healthcare team.
• Evaluate therapy needs and monitor and report patient progress and outcomes.
• Educate patients and family members.
• Prepare timely complete and accurate clinical notes.
• Communicate effectively with patients caregivers healthcare providers office support and other care team members.
