Responsibilities

Position Highlights :

Part of the new Connected Primary Care program

Focus on providing virtual visits

Based in a physical Grafton clinic location with potential to split time at Shorewood

Serve as an extender to the existing care team

Team currently includes 4 physicians and 2 APPs

Program is new and responsibilities may evolve over time

May see patients in person when other team members are unavailable

Flexibility and collaboration are essential as the model grows

Licensure / Certification / Registration :

Nurse Practitioner credentialed from the Minnesota Board of Nursing obtained prior to hire date or job transfer date. Licensure is dependent on state associate works in.
