The role you'll contribute : Responsible for serving as a care consultant and provider for patients with complex wound care needs and ostomy management. This position collaborates with the healthcare team to define and communicate the expected patterns of care for this patient population. Additionally, this role provides leadership for education and research programs to support the advancement of wound and ostomy care and improve patient outcomes through the implementation of evidence-based practice. In addition, this position serves as a resource, leader, and role model for wound and ostomy care, functioning as a liaison with the community, other healthcare professionals, and other disciplines regarding areas of wound and ostomy care. Responsibilities for this role include providing self-directed, effective nursing care to groups of patients and other duties as assigned in accordance with Division of Nursing Practice standards and relevant state regulations. The value you'll bring to the team : Conduct comprehensive patient assessments to determine physical, mental, social, and spiritual needs related to wound, ostomy, and continence care. + Develop and document individualized care plans based on patient assessments, selecting appropriate interventions and supplies, and ensuring accurate records in medical documentation. + Administer treatments and medications safely, monitor patient responses, and adjust care plans as necessary to improve patient outcomes. + Provide direct patient care, including wound care, ostomy management, and continent care, ensuring patient comfort and promoting healing. + Educate patients and families about wound care, ostomy management, and continence strategies, including self-care techniques and lifestyle adjustments. Qualifications The expertise and experience you'll need to succeed Associate's of Nursing Required + 1 Work Experience Required + Registered Nurse (RN) Required + WCC or WCN Required + OMS Ostomy Management specialist OR WOCN Wound Ostomy Continence Nurse certificate preferred.
