We’re seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our team and provide exceptional care to our patients. This role is perfect for someone who values teamwork, professionalism, and making a real difference in the lives of others.

What You’ll Do :

Deliver skilled nursing care under the direction of a Clinical Case Manager

Administer medications and treatments as prescribed

Monitor and document patient health status and progress

Support patients and families with education and care guidance

Collaborate with the care team to ensure safe and effective treatment
