The Care Manager, as part of the interdisciplinary team, assess, plans, advocates, and coordinates care from admission to discharge ensuring a safe transition post hospitalization. Qualifications :

Preferred Experience :

2-3 years acute care hospital setting, preferred

Required Education :

Graduate of an accredited School of Nursing, required

License :

Current RN License in the state of employment required

Education

: Bachelor's Degree of Science in Nursing, minimum required

Certifications :

BLS (Basic Life Support) at hire date, or within 90 days (about 3 months) of hire
