Key Responsibilities
Nurse Practitioners provide primary and / or consultative professional care in a variety of care settings. The Nurse Practitioner will function in collaboration with physicians according to a Collaboration Arrangement. The Nurse Practitioner practice will focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life.
In this role you will be responsible for :
Conducting medical evaluations of predominantly elderly, frail, seriously ill patients in their home environmentDetermining follow up assessments according to patient need and best practice guidelinesCommunicating significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility and other personnel involved with patient careAssessing health status by obtaining relevant health and medical history.Performing a physical exam based upon the age and patient history as per the Standard Care Arrangement with a collaborating physicianOrdering, interpreting and evaluating laboratory tests and other diagnostic studies to assess patient's clinical problems and health care needsPerforming diagnostic of therapeutic interventions consistent with certification, law and state medical privilegesPrescribing and monitoring medications as allowed by prescriptive authority through the appropriate State Board AgencyDeveloping a treatment plan in collaboration with the patient and caregiversReferring patients to physicians as appropriate for consultation or for specialized health resources and treatmentEnsuring continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areasProviding patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visitsHolding case conferences with families when appropriate to advance the care planMeeting minimum productivity requirements based on practice standard.Required Qualifications
Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of NursingMaster's or doctoral degree as a Board-Certified Nurse PractitionerCurrent State License as a Registered Nurse and Nurse PractitionerNursing skills as defined by accepted nursing standardsSit, stand, bend, lift, and move intermittentlyMust be able to lift at least 35lbs and bear the weight of an average adult effectivelyCurrent and valid driver's licenseReliable transportation and valid automobile insurance coverageDemonstrates ability to assess and respond to the needs of patients, families and caregivers in skilled-nursing facility settingsDemonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestylesAbility to handle multiple priorities, documentation requirements and deadlinesDesired Qualifications
2 years clinical experience in a post-acute or as a Nurse Practitioner preferredExperience with geriatric patients preferred.Familiarity with Aprima EMRLocation
