Pinnacle Home Care Floridas largest Medicare-certified home health provider has been delivering high-quality patient-centered care for over two decades and were looking for Speech Language Pathologists to join our award-winning team.

Key Responsibilities

• Evaluate patients speech language and hearing abilities recommending strategies to enhance communication and hearing.
• Perform initial and ongoing patient assessments including OASIS evaluation and documentation.
• Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed.
• Communicate regularly with physicians addressing any changes in patient condition.
• Participate in the periodic revision of care plans updating the physician and care team on patient status and obtaining additional orders as required.
• Educate patients and families / caregivers on speech therapy techniques and related challenges.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Document all evaluation data treatments and patient progress promptly.
• Supervise home health aides every 14 days when speech therapy is the sole discipline involved.
