About the Role :

As a Physical Therapist within the Health Care Services industry, you will play a critical role in improving patients' mobility, function, and overall quality of life through personalized rehabilitation programs. You will assess patients' physical conditions, develop tailored treatment plans, and implement therapeutic interventions to address a wide range of musculoskeletal and neurological conditions. Your expertise in orthopedic rehabilitation and manual therapy will be essential in helping patients recover from injuries, surgeries, or chronic conditions, enabling them to regain strength and independence. Working closely with patients in various settings, including home health environments, you will provide compassionate care and education to support long-term health and injury prevention. Ultimately, your efforts will contribute to enhancing patient outcomes and promoting active, healthy lifestyles across diverse populations.

As a Physical Therapist, you will :

Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.

Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.

Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.

Applies concepts of infection control / universal precautions in performing patient care

Assure continuity of quality patient care delivered with appropriate documentation

Provide effective communication to patient / family, team members and other health care professionals

Promote agency philosophy and administrative policies

Maintain and complete all documentation within company stated guidelines.

Requirements

Successful completion of accredited physical therapy program and state licensure exam.

Valid Physical Therapy license

1 year of home health experience preferred

CPR certification

Valid Driver’s license

Valid Auto Insurance

HomeCare HomeBase experience preferred

Physical Demands

Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

Must be able to lift, carry, pull, or push up to 100 lbs..
