The Unified Government Public Health Department is seeking Family Support Specialists (Program Coordinators) for its Healthy Families Wyandotte (HFW) Program.

HFW is an evidence-based, intensive home visitation program. The Family Support Specialists are responsible for initiating and maintaining regular and long-term (up to 3 years) contact / support with families. This activity will occur primarily within the family's home; each visit should last one hour. The interventions should be family-centered, strength based and directed at : establishing a trusting relationship; assisting in strengthening the parent-child relationship; assisting parents in improving their skills to optimize the home environment; improving the family support system; and increasing the family's ability to problem solve and assume the role of advocate for themselves and their children. The activities may also include identifying and referring for contact / appointments at other supportive agencies, including health care appointments. The Family Support Specialists will also be responsible for assisting the family in establishing goals and a plan for the accomplishment of those goals, as well as the assessment of the normal growth and development of the target child. Must be able to work with a diverse client population, work in a team setting, maintain an appropriate caseload, complete documentation on time, exercise good judgment, and refer clients to community resources as appropriate.

Qualifications : Bachelor's degree in social work, nursing, special or early childhood education, child development or a related field, experience in working with or providing services to children and their families, or a related field and two years of related : or any equivalent combination of education and experience sufficient to successfully perform the essential duties of the job. Must be able to work with a diverse client population, work in a team setting, maintain an appropriate caseload, complete documentation on time, exercise good judgment, and refer clients to community resources as appropriate. Bilingual in Spanish / English or Burmese / English is a plus. A valid Kansas driver's license and reliable transportation are necessary. Must reside in Wyandotte County or relocate into the county within 12 months. This is a grant-funded position.

Salary Range : $26.74-$28.95 / hr.
