job_description.job_card.job_descriptionProvide, coordinate, and direct home nursing care according to physicians' orders and agency policies Responsible for supervising and evaluating Home Health Aides and Licensed Practical / Vocational Nurses Conduct home visits and attend periodic office meetings Manage an average of 5-10 patients per day, equating to roughly 6-8 productivity points Handle cases such as Start of Care, Wound Care, and Follow-up Visits Complete all documentation within 24 hours of patient appointments Collaborate with the Clinical Manager for patient care and workflow Be open to on-call rotations as per location and profession requirements Engage with weekly regional meetings in assigned territory covering specified county areas Achieve a weekly productivity goal of 30 points Have access to another clinic to float to if necessary for additional patient engagement

- Reliable transportation, a valid driver's license, and auto insurance
- Willingness to travel up to 75 miles from the home clinic
