BE APART OF LUTHER MANOR'S HOSPICE TEAM
FULL TIME RN CASE MANAGER POSITION AVAILABLE
The Luther Manor Hospice RN Case Manager gives skilled nursing care to patients in their homes; administers medications and treatments.
- Uses tools of observation, interviewing, data history, and physical observation and assessment, to collect and document basic patient data for initial assessment.
- Visually observes and interprets changes in behavior, psychological and physiological status of patient for ongoing assessment.
- Develops nursing diagnosis and care plans individualized for each specific patient and modifies care plan based on ongoing assessment of patient condition.
- Evaluates patient compliance with self-medication and self-treatment, and modifies care plan accordingly.
- Contacts patient or family to set up schedule for home visits.
- Gives or directs total care for all patients including personal hygiene, comfort measures, nourishment, and provides physical and psychological support to the patient and family.
- Documents medication given, care given, patient condition, assessment, and patient diagnosis on patient charts.
- Maintains integrity of the patient chart
- Directs and applies problem-solving techniques in resolving issues.
- Drives to and from patients' homes and to meetings at other locations as necessary
- RN license to practice nursing in Wisconsin upon hire; valid Wisconsin driver's license upon hire; re-certification as required.
- Minimum of 2 years public health, home health nursing or hospice experience (preferred).
- Prefer 4-year bachelor degree in nursing or 3-year diploma. Will consider 2-year associate degree with significant experience
- Possesses outstanding interpersonal skills, including excellent written and verbal skills.
- Ability to effectively work with and through physicians, subordinates, customers, and other personnel in a cooperative manner in order to improve clinical quality outcomes.

- Ability to communicate in a manner that is accepted and understood by staff, residents and families.
- Ability to maintain confidentiality and professional conduct in all work-related and / or emergent situations.
- Thinks critically; utilizes sound judgment and is a team player.
- Good health and physical stamina and the ability to make in-home / facility visits.
- Experience working with computer systems; proficient in Microsoft, Excel and Word.
- Must have a valid Wisconsin driver's license and good driving record Must have auto insurance with Luther Manor minimum coverage requirements.
