The Foster Care Case Worker plays a vital role in our mission to deliver coordinated individualized care to our communities.

Build Your Purpose.

As a Foster Care Case Worker you will provide :

• Case Management and Support :

Conduct interviews with children and families and complete detailed home studies.

Develop individualized treatment and service plans that address each childs unique needs.

Provide counseling and guidance to children and families throughout placement or discharge.

Child and Family Supervision :

Oversee children in foster resource and adoptive homes as well as independent living arrangements.

Perform regular assessments to determine the childs needs for medical psychosocial educational and nutritional services.

Service Coordination and Continuity :

Link children to appropriate services minimizing barriers to care and coordinating with other agencies to ensure comprehensive service access.

Assist in arranging referrals for medical remedial and dental services as outlined in each childs treatment plan.

Monitor the ongoing progress of each case evaluating the effectiveness of the care plan and collaborating with involved parties including family assessment teams.

Education and Counseling :

Build a supportive trusting relationship with the child promoting growth and well-being in alignment with the service plan.

Educate children and families on available resources and work closely with team members to prepare comprehensive case plans.

Qualifications

Qualifications :

Education :

Masters or Doctorate in Social Work or a related field (sociology psychology special education or counseling) from an accredited institution or

Bachelors degree in Social Work or a related field with relevant experience in casework services.

Experience :

At least one year of experience in providing casework services to children and families with a masters degree or

Two years of experience in casework services with a bachelors degree in any field.
