Mental Health Professional

Pilsen-Little Village Community Mental Health Center Inc d / b / a Pilsen Wellness Center Inc. seeks a mental health professional to support patients and psychotherapists in the implementation and continued adherence to each patients treatment plan. Plan and evaluate patients behavior as between individuals and within the group. Guide and counsel patients in their on-site residential maintenance including but not limited to bed making cleaning bedrooms personal hygiene and washing dishes. Provide assistance to patients in crisis and emergency situations. Participate in weekly team meetings to coordinate patient care with psychotherapists and other personnel. Conduct building rounds to ensure patients safety and security. Participate in monthly online training with the State of Illinois. Document each patients progress within the Community Innovation Service software system including tracking important events medication changes treatments and patients symptomology. Comply with State reporting requirements including updating patients IM-CANS profile with the State of Illinois and filing periodic reports under Illinois Medical Rule 132.
