• Already have established, active relationships with hospital discharge planners, case managers, hospitalists, or social workers in the Atlanta metro who have sent you referrals in the past 24 months.
• Willing and able to do daily in-person visits to hospitals, SNFs, ALFs, and senior living communities (100 % field-based – no remote / hybrid).
• Own a reliable vehicle.
