Basic Function :

Under the general clinical and administrative supervision of the Project Director, this unique position will provide trust-based information and education, help clients cope with and overcome fear, walk clients through the often complex healthcare system, and help them strategize to manage the logistics of caring for oneself in the context of a complicated life.

Nature and Scope :

In contact with a variety of Howard University and Howard University Hospital faculty, students, residents, staff, and support staff in various departments and community-based case managers, other professionals, clients, families, and individuals.

The incumbent will promote HUH CARES and CIDMAR programs, offer resources and referrals to available community programs, and provide pre – post counseling and testing.

Outreach will be conducted within District of Columbia communities that are considered high risk for HIV / AIDS and STD's, and also have high prevalence rates for HIV / AIDS.

Principal Accountabilities :

  • Screen patients for eligibility in service programs.
  • Identify individuals who know they are HIV-positive but are not in HIV medical care by receiving referrals, utilizing organizational information about missed appointments, and conducting outreach at various community organizations and venues;
  • Support and increase linkage to and retention in care and adherence to treatment by educating clients about treatment and the appropriate use of services.
  • Provide psychosocial support services with status neutral program services.
  • Conducting Outreach Support linkage to and retention in care by re-engaging clients lost to follow-up.
  • Serve as a frontline public health worker who is a trusted member of and / or has an unusually close understanding of the community served.
  • This trusting relationship enables the CHW to serve as a liaison / link / intermediary between health / social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
  • Builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities, such as outreach, community education, informal counseling.
  • Enhance HIV care teams by working in partnership with case managers, nurses, doctors, social workers, and other service providers to address the medical, social, and economic needs of people living with HIV (PLWH).
  • Serve as a bridge between the patient, the community where the patient lives and medical clinics or community-based organizations bi-directional.
  • Improving the health of patients and their communities and they also influence the program and the clinical setting in which they function.
  • Provide accurate information to clients about living with HIV, preventing further transmission, the benefits and challenges of HIV care and treatment, and available services and how to access them;
  • Provide personalized assistance to clients to help them enroll or re-engage in medical care;
  • Together with case managers and clinical staff, support clients throughout the early part of their care so they participate fully, become educated, and can advocate for themselves;
  • Comply with all program protocols, including those regarding confidentiality of client information;
  • Complete employee orientation and all mandatory program trainings to guarantee appropriate competency in program requirements.
  • Keep supervisor informed of all problems, challenges and conflicts related to job; and
  • Make referrals to individuals, who may be underserved to HUH CARES and CIDMAR centers for primary medical care, housing, drug treatment, mental health counseling or other care as needed.
  • Provide education on various services available within the city so as to encourage participation through utilization of services
  • Participates in staff meetings and training as necessary to improve services offered to District of Columbia residents.
  • Maintains knowledge of programs and resources available to District of Columbia residents that may enhance the health care of those residents.
  • Provides consultation services to the HUH emergency room staff on the benefits of referring eligible clients to Ryan White-funded services.
  • Maintains a monitoring system of record keeping assessing the number of educational and preventive materials distributed to community members.
  • Performs other duties as assigned.
  • Promote adherence to the Health Sciences Compliance Program, the Howard University Code of Ethics, and the Health Sciences Standards of Conduct.
  • Attend annual and periodic mandatory Compliance Program training, including the Health Insurance Portability and Accountability Act (HIPAA) Privacy training.
  • Participate in activities that promote adherence to Federal healthcare program requirements.
  • Actively participates in Health Sciences Compliance Program activities.
  • Adhere to the requirements of the HIPAA Privacy Policies and Procedures.
  • Maintain confidentiality of patients, their families, and staff.
  • Minimum Requirements :

    A baccalaureate degree in a health-related field, or 5 years of experience in community relations. Ability to establish a good working relationship with co-workers, supervisors, and clients. Initiate contacts with residents of communities, developing long-lasting relationships. Ability to communicate effectively and research community resources. Ability to learn information relating to HIV / AIDS and Sexually Transmitted diseases, including their origins, signs, symptoms, and treatment. Ability to use a computer.
